Access Statements

1. I’m applying for an Access Statement, do I need to contact my GP?

If you have a specific learning difference as ADHD, an autistic spectrum condition, a physical, sensory or long-term health condition, or a diagnosed mental health condition, you can ask the GP for an Access Statement Medical Evidence form to be completed on your behalf.

2. How do I request medical evidence from my GP for my Access Statement?

You can get in touch with us online by submitting a Patient Triage Request or in person to request medical evidence for an Access Statement. Your request will then be passed on to the relevant member of our team to process. A GP will then complete an Access Statement Medical Evidence form on your behalf, based on information from your medical record.

3. How long does it take for the GP to complete?

We action Access Statement Medical Evidence forms within 21 days of receipt. There is no charge for these. We cannot complete these forms urgently. We will contact you once the form has been completed.

Disabled Students’ Allowance (DSA) Forms

1. I’m applying for a DSA, how do I get medical evidence?

You will likely need to provide medical evidence of your long term health condition to your university in order to apply for a DSA. Student Finance England (SFE) have created a form for you to take to the GP or other medical professional. You can get the DSA form from the NTU website below.

2. I have the DSA form, how do I give it to my GP?

Please submit the form to us online via a Patient Triage Request or hand it in to the surgery. Make sure your personal details are completed on the front page. Once you have submitted the form to us, it will be passed onto the relevant member of our team.

3. How long does it take for the GP to complete?

We action DSA Medical Evidence forms within 21 days of receipt. There is no charge for these. We cannot complete these forms urgently. We will contact you once the form has been completed.
