You can register by filling in an online registration form and health questionnaire.
Please complete the registration form by clicking ‘register online’. Once you have completed a registration form you will be registered at the practice within 24 hours. You will then be able to access our services like submitting medical or admin requests via our Patient Triage Service. This is an online consultation system where you can send medical request to the care navigator team and we deal with these requests on the day.
Please register with us closer to the time of your arrival in Nottingham or near to the time of your course start date!
If you would like to fill in a paper registration form please come in to the Student Health Centre and ask for one.
